Scouts and Scout Parents,
At a meeting on Saturday, January 9, 2010, Mr. Schenk and I were informed of the NEW BSA Health and Medical Record Requirements. The requirements are for everyone, YOUTH and ADULT. The form is a four page document which contains the following:

Page 1 is the Medical Information page, which includes the “Instructions”
Page 2 is part A, filled in by the parent/guardian, and is the “Health and Medical Record”
Page 3 is part B filled in by the doctor, and is the “Physical Examination”
Page 4 is part C, filled in by the parent/guardian, and is a hold harmless agreement, adult authorization/NOT authorization, and “signatures” You may use my name if you like on the authorization; (David M. Shields (Scoutmaster), 516 593-4254, (c) 516 662-7206)

Page 2 (A) and 4 (C) are required for all BSA members, for participation in all activities that do not exceed 72 hours, where the level of activity is similar to that normally expended at home or at school, such as day camp, day hikes, swimming parties, or an overnight camp, and where medical care is readily available.

Page 2 (B) is required with part A and C, for any event that exceeds 72 consecutive hours (Summer Camp), or when the nature of the activity is strenuous and demanding, such as a high-adventure trek. Service projects or work weekends may also fit this description.

I have included the link above to open the form. The form is user friendly and can be filled in online, and saved on your computer. Please fill our pages 2 (Part A) and 4 (Part C), print it out, as one page if you can, Part A on one side, Part C on the back, sign it and bring it to Mr. Shields or Mr. Schenk at the next troop meeting or soon afterward. Page 3 (Part B) will be needed in order to go to summer camp, and you can do that annually when you have your sons annual physical for school or summer camp, and is good for one calendar year. For the sake of convention, please date your form “January 1, 2010″, as we will need this to be done annually for each calendar year. We are sorry for any inconvenience, but this is the new BSA policy, and it is what it is.
